A comprehensive approach to refractory cardiac arrest: saving more lives one way or another

چکیده
According with the last updated guidelines on resuscitation, the underlying cause of cardiac arrest (CA) should be identified, treated and, if possible, reversed with different strategies but a common target: to increase long-term survival with good neurologic recovery. At the same time, some countries have implemented protocols for donation after considering the irreversibility of cardiac arrest and the failure of resuscitation attempts. Both strategies are complementary and should coexist. Thus, we would be able to go beyond the refractory CA firstly and, if not indicated or unsuccessful, we could increase the organ donation pool after confirming irreversibility. International recommendations on resuscitation (1) highlight the importance of high-quality cardiopulmonary resuscitation (CPR) based on minimal interruptions, focused on determining the cause of the cardiac arrest and offering, as early as possible, etiological treatment of potential reversible causes. Several pioneering protocols have been developed throughout the world in order to provide a multidisciplinary approach to the out-of-hospital (OHCA) and in-hospital cardiac arrest (IHCA). Moreover, international programs including non-conventional resuscitation procedures (NCRPs) have been set up (2-7).

The Efficacy of Propofol and Midazolam in Treatment of Refractory Status Epilepticus in Children
Background: In this study, we compared the efficacy and safety of propofol and midazolam in treatment of children’s refractory status epilepticus. Methods: We recruited 32 patients with refractory status epilepticus. Of those, 16 were treated primarily with midazolam and 16 received propofol. Results: We achieved complete seizure control in 6 (38%) patients treated by midazolam, and in 10 (63%)...

BACKGROUND Sudden cardiac death is one of the most frequent causes of death in Germany and the third leading cause of death in the industrialized world. Yet, the percentage of people providing first aid in the case of sudden cardiac arrest in Germany is alarmingly low by international comparison. Training Germans or reminding them of the simple but effective steps of resuscitation, so that ever...

This condition poses an immediate threat to life and is always considered to be a medical emergency. In Western countries, cardiac arrest is continuously highlighted as one of the leading causes of in-hospital death and the latest research suggests that surviv...

Currently every day in the USA 1000 people die of sudden cardiac arrest (SCA) outside of hospitals or ambulances before emergency medical help arrives in the streets, workplaces, schools and homes of our cities, adults and children. Brain death commences in 3 minutes, and often the ambulance just can’t be there in time. Citizen cardiopulmonary resuscitation (CPR) and automated external defibril...
متن کاملA Case of Cardiac Arrest after Topical Phenylephrine Administration in Adenoidectomy Surgery
Some otolaryngologists administer topical phenylephrine for bleeding control in adenoidectomy surgery. Absorption of this drug from surgical site can lead to increase in blood pressure due to vasoconstriction and then bradycardia related to baroreceptore reflex. Our case was an intraoperative arrest of a 9-yrs-old girl related to administration of topical phenylephrine during adenoidectomy how...
